We accept most public and private insurance plans. We also offer special pricing for cash-paying patients for most procedures.

We are not accepting NEW patients at this time. If you would like to be put on a wait list please go to our NEW PATIENT tab complete the new patient paperwork and mail or fax it into our office, as directed, and we will contact you once a spot opens up.

Patient Portal is for CURRENT patients who need to make an appointment with Dr. Osipchuk, check on labs or medications. This is NOT Telemedicine link.

TeleMedicine is for CURRENT patients who ALREADY have a TeleMedicine Appointment.

Make Payment Online is for CURRENT patients who need to pay on a balance, need to pay copay for upcoming appointment or self pay patients who need to pay their monthly fee.
